Usually because grades were set flat or the surface has settled. The fix can be anything from adjusting a catch basin to regrading and repaving a section. We'll find the cause at the site visit.
They're Ontario standard crushed stone base materials. Granular B is the coarser subbase and Granular A is the finer base right under the asphalt. Depths depend on soil and traffic.
